Is the document easy to read?   Does the document make clear recommendations?   Is the literature review adequate?   Is the evidence grading adequate?   Are UCSF specific "process" issues addressed?   Have key outcomes been identified?   Are mechanisms in place through which these outcomes can be monitored?   Has an implementation plan been developed?   If in use, how has guideline been working?   If in use, are all interested parties aware of guideline?
